I was diagnosed with sleep apnea a few years ago and since then I have been on a quest to improve my sleep. Recently I had a visit with Dr. Tan to do the Nightlase laser procedure to help improve snoring. The procedure was quick and easy. He was very thorough in explaining how the laser works and made sure I was comfortable during it. I can’t wait to see and feel the results of better sleep. Was really looking forward to working with them as I have had jaw pain for the last 9 months. However when it came to insurance, I finally received work insurance and wanted verification it would be accepted. Never heard back, so day before my appointment I reached out again. It wasn’t accepted and it was a $400 new patient visit without it.
I said I needed to cancel and was told they understood and it was okay. An hour later I’m charged $50. I called back and spoke with both reception and dr and was told it was a misunderstanding on my end since they gave me recommended insurance plans. Had I heard back in a timely manner, this wouldn’t have happened. I feel a little played. Unfortunate. Just know if you go here I’m sure you will receive great care and customer service just don’t bring up any issues you might encounter because then that will change.

As a first-time patient at Form & Function Dental Aesthetics, I can honestly say this was the best dental experience I've ever had.

From the moment I walked through the door, I felt genuinely welcomed. The office itself feels more like a spa or an art gallery than a dental office. It's beautiful, calming, and thoughtfully designed.

Dr. Tan was exceptional. He never made me feel rushed and took the time to thoroughly explain everything, answer all of my questions, and make sure I understood what we were looking at. You never get the feeling that you're just another appointment or that he's trying to get you in and out as quickly as possible. That level of care is rare and incredibly appreciated.

The rest of the staff was just as wonderful. Once I was in the exam room, I was handed a beautiful acrylic menu with comfort options that included a weighted blanket, under-eye patches, AirPods Max headphones...or all three! What dental office offers that kind of experience? It was such a thoughtful touch that immediately put me at ease.

I also loved how comprehensive the exam was. They took an incredible number of photos of my teeth and mouth, then sat down with me to review them one by one. It was fascinating to actually see and understand what was going on instead of just being told everything looked "fine."

This practice has completely changed my expectations of what going to the dentist can be. I honestly don't think I'll ever be able to go back to a regular dental office again. I've had 2 appointments with Dr. Tan. The first was a full assessment including airway. He lent me a special pulse oximeter to track my breathing during sleep. I'm looking forward to the results. I had my crowns and a filling during the second appointment. Dr. Tan made sure I stayed comfortable during the procedures. He even gave me higher grade anesthetic, a weighted blanket, and light blocking sunglasses (due to my light sensitivity). And his assistant, Heavenly, is really nice. Just walking through the door, I knew this was going to be a dental experience like I’ve never had before. The design and esthetic of the front space is like stepping into a calming spa, and has a welcoming fragrance that helps to put you at peace. Was welcomed in and given a full tour of the entire office, and again I cannot get over what a top notch experience it gives, not to mention how impeccably clean, organized, and sanitary everything is. Then I met with Dr. Tan in a consultation room, to really discuss the reason I came in for, my teeth. He really took the time to ask in depth questions and have realistic answers of what I’m wanting. Something I value in my day to day life and in my career is finding the “why’s” and he brought that into our consultation. Not once did I feel that I was being rushed to make a decision right then and there, but to allowed to think about things and my path to my goals. I was out of state when one of my crowns came off, I was able to get an urgent appointment with Dr Kevin Tan within 2 hours by the referral of my family dentist.
I was given Dr Tan’s personal phone number instead of his office phone number, but Dr Tan nicely answered my call and told me that he could help me that afternoon and told me to call the office so I could be on their schedule.
Dr Tan worked on cleaning up the decay on the surface of the tooth then built up a preparation for my family dentist to put on the new crown when I come back home.
Dr Tan is really polite, not with just an old lady as I am, but I could hear that all his conversation with the Assistant were always ended in “, please!”.
The next morning, Dr Tab followed up with me to make sure that I didn’t have any issues after the numbing medication wore off, and if I had any uncomforted feeling while eating…..

I am really fortunate to be able to meet and be taken care of by Dr Tan!

I would choose Dr Tan as my family dentist if I happen to be a Nevada resident some day and I strongly recommend everyone to give Dr Tan a visit to enjoy his professional manner and skill.

Dr. Tan was super nice and welcoming, and even addressed my dental anxiety that I listed on my intake form and was super respectful when I asked for him to explain things in a way that didn't scare me. He ended up recommending a few treatments and then orthodontia. He referred me to one of the best orthodontist in the city and now my teeth are super straight and literally no dentist before had told me that!
