We arrived late at night. The hospital did a terrible job on the discharge. The nursing staff was extremely attentive and made every effort to figure out the patient’s comfort and medical needs, medication, and pain management. We’ve experienced genuine concern and recognize the extra effort to provide the best care. Our patient has been in and out of multiple SNFs and this is one of the best. Food is better than just about all others. The nursing staff and admin extend politeness, respect and regard, at the highest level. Our patient’s situation has been complicated and messy because the hospital abdicated their responsibility for a safe discharge. The Green Valley Health and Wellness staff have been compassionate and caring leaders as they help us navigate this situation while taking care of our loved one. We are most grateful.

I was looking to get my father admitted at this place because it seemed to be clean and well-maintained. The front desk receptionist she was kind and professional, but the person I spoke with who’s in charge of admitting and the higher up management were absolutely terrible unprofessional and uncaring, I hope whoever owns this place sees this message because they need to know what is going on with their upper management when I spoke with these people and asked them if they had a bed open which I was told that they did they said no we don’t in a rude manner and hung up the phone before I could say anything else. Do not bring your loved ones here they will not be taken care of!

My mother is a resident at Green Valley health and wellness suite. We are very impress and happy with their services. The Administrator, Mr. Bill Ward, and case worker Tracy Figueroa are amazing. Always approachable and ready to help if needed. The facility is well kept, and very clean. Nurses and staff are very friendly and knowledgeable. Elijah a member of the cleaning team is an excellent young man who cares about my mother’s room cleanness. We are very grateful for all that they do for mom. Thank you for an awesome job everyone.

A Heartbreaking Experience and Deep Concern for Resident Care

My experience with this nursing and rehabilitation facility has been profoundly disappointing and emotionally difficult. I expected a compassionate and attentive environment for residents, but what I observed left me feeling deeply concerned. During my visits, the facility often had strong, unpleasant odors, and many residents appeared unattended for long periods of time. It was painful to see individuals who deserve dignity and kindness not receiving the level of care I believe they truly need.

I repeatedly attempted to communicate my concerns to staff, but I felt that my worries were either minimized or not addressed in a meaningful way. As a family member who wants the best for their loved one, this was incredibly upsetting.

I sincerely hope this facility takes steps to improve the quality of care and communication provided to residents and their families. Until then, I strongly encourage anyone considering this location to visit in person, observe carefully, and ask thorough questions before making a decision.

For my review on Green Valley, I will say it is a three and a half for rating. Now, there is not a half star to choose from, so I will say it leans more towards a four star, than a 3 star. I only had one nurse say my antibiotic was running and it was not. What alerted me that it was not running was the lack of a drip. I learned from Harmon to observe everything concerning the antibiotic machine. I also learned enough to operate it myself. With that being said, the first time this particular nurse said the I.V. was running, I kept it to myself that I knew she was lying. I waited a few more times that she worked to see if she would do it again. Sure enough, the line was clogged another time and she did the same thing. For some reason I said nothing to anyone and resolved the issue myself. That is about all the wrongs I can claim for Green Valley. I believe the nurse had racism issues. Her and a couple others with that problem. We should never have a problem with someone because of the color of their skin.

After getting a horrible surgery from Henderson hospital and it causing me to be bed bound for 2 months I found heaven in this facility. Bill War is administration and he is constantly walking around to make sure people feel comfortable and things are running smoothly with their individual recovery plans. Daisy is an awesome case manager and Cesar is like a friend as he transports you to your appointments
And helps keep supplies stocked.
Eugenia is a God send of a nurse who focuses on the best for her patients, Silvia and her other coworkers of CNAs have a lot of love to encourage their patients with positivity to promote joyful days
Ashley the CNA validates her clients with respect and clear communication
AJ in physical therapy takes time to see his clients for the individuals they are to challenge them in healthy physical therapy.
Their house keeping crew keeps the building spotless so there is no cross contamination.
I had no family support and needed a time out in a safe facility to be guided in recovering my strength that Henderson hospital took from me with a contaminated surgery.
I am not longer bound by my circumstances because of Green valley rehab and they taught me through love that it's ok to trust medical professionals to get the care they need.
And Marissa is a blessing from ocupational therapy to help someone get the assistance and personal equipment needed to recover their own power in their personal hygiene.
Definitely a place to check out

My son was a patient there for two weeks. We were treated as though we were the only ones there. The love felt from the staff and the care we received was great! If I tried to remember everyone's names, l would miss someone. Each day we asked the staff if they were back the next day. We would have kept them all. Every need was met. We tried to do all we could to alleviate the requirements of staff to chase all of the needs, but any time we needed anything, we did not wait long. They were very busy, and often short on staff, but they treated us as though that was not a problem. We would recommend this facility for others who need it. PS- the food was good too, my son had no complaints.

When I first arrived, I felt decent, however during the night, things became disruptive. Patients who clearly needed mental assistance screamed in the halls during the night. During the night my service was limited and hardly anyone tended to me. These were mainly people from other agencies filling in needed positions. The actual employees were lovely. During the day I was cared for accordingly. Whenever I rang the bell, I could expect to be attended to within the hour and I was checked on periodically throughout the day. The facility itself was not dirty at all. No smell and quite clean halls. The rehab and physical therapy staff were especially excellent. However, because the facility was understaffed, the temporary employees often didn't want to work and mostly talked amongst themselves instead of tending to patients. It was only certain people who made things bad but overall the facility was on par.

I would like to give a special shout out to Nate, Ceasar, Chad, AJ, Cassidy, and Maritza and Carmesha. They went above and beyond in service.

My father is here and if you need paperwork to move your loved one?? forget about it the caseworker will not get back to you at all and seems to never work and doesn’t work fridays. your loved one complaining they’re not getting their medication or a shower? don’t think about calling because they either a) won’t call you back or b) transfer you and then get hung up on. completely useless and just seems like they want to suck your loved own set of money. And that number the “owner” keeps giving out for 1 star comments?? yea that’s the number that no one ever freaking answers don’t give me no damn number if no one useful is going to answer! if you want results do not let your loved one go here EVER!

I believe the staff is taking care of my friend in a caring and respectful manner. Everyone seemed kind, welcoming, and friendly. I would like to comment that the hallways smelled strongly of urine, noticeable on the way in as well as as we left. Maybe this is something the staff is used to, but these two visitors were hit with it and it was nauseating. This is an unhealthy environment when it is circulating in the air so strongly.

I had to stay here after a bad pelvic fracture and surgery. The administration went out of their way to make sure I was taken care off and had everything I need. The nurses were always pleasant and responded timely to my requests and medication schedule. Finally, the PT/OT therapy team was incredible. It was a joy to work with them each day, and they were always clear with their instructions. Their therapy plan helped me to recover significantly faster than anticipated.
No one wants to go to a facility, but if you have to, this is the one to go to!

It is amazingly difficult to get through on the phone to talk to patients 🤯 My loved 1 is in the final stages of dementia and it can take days before I'm able to get a hold of them! It's horrible not knowing how they're doing. It's also horrible that when I eventually get through I don't get to talk to them and has me wondering if they just don't want to bring them to the phone. When I do get to town and there, I realize how understaffed they are. I hate thinking that management or the owner(s) would put people through this constantly just to save $!!! Most care about the facility NOT it's patients!!! Why be a nurse just to watch them suffer & not help!?!?

This facility, believe it or not, is beautiful, but the staff there pretend to love what they do. My wife got there around 11:30 p.m., which they knew she was on her way. Her sheets had urine stains on them, and the garbage can was full. After I mentioned both things, they were changed immediately. They were notified before my wife got there about the medication she would be needing so she would feel comfortable, but they overlooked that as well. My wife still hasn't received any medication, and it's 11 a.m. The owner of the hospital will smile in your face and pretend they're helping you, but they're just trying to keep the facility full to make money. She never once showed her face until they heard me show up and was taking my wife home. My advice to anyone who goes here is to have a loved one close by to check in on you because the staff there will ignore you until they feel like coming.

During the last 2 weeks that my dad has been at Green Valley Health and Rehabilitation I have witnessed a great deal of care for my own father but others as well. Questions that have been asked are always responded to immediately and with honesty. No one is happy to have to be in the situation of needing skilled care, or being the family of the person in skilled care, but I have witnessed staff trying to make everyone feel as well as possible and treated with kindness. I cannot tell you how fabulous this place and all its people are,from the 1st time my brother went into this place,the people always smile,talk pleasant and offer to help you if you look puzzled,highly,highly recommend this place for yourself or your loved ones!! I don't have a bad word to say about this place or its people,I'm just sad to not see all their smiling faces 5 to 6 times a week,my brother really enjoyed the ability to chose what food he got and the ability to talk to CNA's and the nurses without judgment,this facility is always clean ,bright and bustling with people always moving and helping others. Great place,great Nurses,great CNA's ,highly recommend them Greenvalley health and wellness suites ,has the best PT,OT,and Joyce in would care is the best in her field ,I've met a few in this journey and she is always willing to talk to you and also listen to you! The Physical therapy department is very good at getting people up and moving,my brother lives in AZ so we are moving him closer to his home ,im a little sad as to not be able to see these ladies/ nurses and CNA's ,pt department people,occupational therapy department anymore 😢 they are very good at what they do!! ❤️💙

Wow, you know a place is going to be bad when you're told on the phone not to worry about the rating on here. If I could give them negative stars, I would.

First this facility is SEVERELY understaffed. There's never any employees at the front office, every "nurse" is running around like a chicken with it's head cut off and they act like highschool kids hiding and talking amongst themselves (sometimes about you or your loved one). They do not follow any sort of protocol, except their own and it's scary. No check ins, people come and go without anyone getting screened. I've never seen anyone to talk about plans or progress. It's been four days here I am legitimately scared for my loved one and will be transferring them out ASAP.

EVEN THE TRANSPORT PEOPLE WARNED ME ABOUT THIS PLACE. Seriously, this is the scariest facility I've ever been in and I hope to be rid of this place soon.

Update: cannot seem to talk to anyone here from administration. It's like they are non-existent. When you call on the phone an angry lady will answer the phone like it's a problem you called and most likely hang up on you. This place is absurd. The amount of absolute neglect and abuse I've seen from the limited time spent here is more than enough to get a place shuttered in California. I've witnessed a few patients screaming in their rooms for hours only to be found on the floor and then spend thirty to forty minutes on the ground until a cna is found to touch them, the staffing is mostly substitutes from fill in agencies, no one knows anything about the patients they're working with, sometimes not even their name. A LOT of red flags with this "facility" that is more akin to a sanitarium than a rehab. I foresee a great deal of lawsuits coming from the "care" received here and it's disgusting. I couldn't even imagine leaving someone here, anyone who does deserves to go to prison. It's neglect and this level of care should not be tolerated.

I just want to personally thank D!!! D you were beyond amazing and the way you treated my dad (Robert) like a human and showed compassion meant so much to us both. I was sad moving my dad out only because of D, but needed him closer for his final days. There was only one nurse who was always very rude to my dad and myself.. so I do hope this was addressed. I just asked the hospice company to educate her on dying individuals and I didn’t want her to be reprimanded or get into any trouble. It’s never okay to make comments to someone’s family saying i don’t know why you called hospice saying he’s not eating when I had physical proof he hadn’t for days. And thankfully his appetite appeared for a few more days and he was able to get some of the foods he loved. Then the guy nurse saying we brought him junk when it was his favorite tacos.. my dad was dying and tacos wasn’t going to change that. So please refrain from such comments in the future. Remember try and pit yourselves not only in the patients shoes but the families… my dad passed away peacefully Monday. Thank you D for being the one to take such amazing care of him. I can never thank you enough for your kindness and compassion. The facility is absolutely stunning and clean.

Green Valley Health and Wellness Suites is a clean, beautiful “Marriott”-like hotel and everyone who works there (from Admissions staff to nurses, CNAs and case managers, business office and literally everyone) has been excellent, delivering professional, compassionate care to my father, consistently every day. We come from medical and healthcare backgrounds, and we not only “approve” of this facility, but we highly recommend it if you want you or your loved one to receive top-notch care. Special thanks to Admissions Director Darren for making the admissions process smooth with his expertise and attention to detail. And thank you nurses Joyce, Victoria, Tu, Vanessa and Regine and CNAs Gloria, Paulette, Irena, and Denise. August 17. Im currently getting bad attitudes from several staff members. My insurance stopped paying for my care. I am paying $500 per day. My meals are often wrong. They are hardly ever served hot. Medicine is not given on time and when asked for it. Nurses say you have to wait your turn or they say ill get to you soon while having a bad attitude. I am often talked down to like im a 4 year old. As if I don't know anything about my needs. Agency staff do not care about patients, they are here to just do as little as possible and collect a check.

My grandma came here from the hospital for some care/therapy. She’d call for help but get ignored. However, there would be multiple nurses standing around in the hallway doing nothing or just walking around. They would not bring breakfast, help her get up, & pretend they don’t understand her because she doesn’t speak english. The first day she stayed here they helped her shower & she fell on the floor cos she couldn’t walk. The nurse couldn’t help her get up, other nurses who were called for help just did nothing. My sister had to go in herself & pick her up & strained her back. 4 days passed, still no update on when therapy would even start. Don’t recommend this place at all. Many patients stay here & are forgotten they won’t even take them outside of their room… On top of this all they said was sorry and they’re busy..? Worst representation of hospitality! Do not bring people you care about here please!!!

I do not recommend this place to anybody! This is the worst facility ever.
My grandma had to stay here for a couple of days and the nurses were nothing but rude to her.
They would yell at her when she would ask for something or would say they dont speak english as an excuse but wouldn’t even attempt to try to understand her. They would also take forever to get back to her whenever she needed something.
On the first day, one of the nurses showered her and my grandma fell to the ground keep in mind my grandma cannot walk due to her infection and the nurse could not get her up. Luckily, my older sister was visiting at the time this happened and had to help my grandma by herself because the nurse could not get her up. The nurses here do not seem experienced at all and do not seem to care about their patients. It seems like they just want to keep their
patients inside the facility for money and nothing else.

I am a patient here at green valley health and rehab and im having a problem with getting my social security restarted. But there is a man here named Bill War that helped me with getting some nintendo switch games. He didnt have to do this but he did it out of the goodness of his heart. I wanted to trade in a bunch of games and probably wouldnt have gotten what near what they were worth and when Bill heard this he just gave me 2, 20 dollar cards. That was really nice of him and i hope he knows he really made my March that much better. Thank you Bill very much.

This is place is trash. My grandmother was there and the first day she fell while the cna was bathing her and my grandma screamed I ran to the bathroom and found my her on the floor the nurse couldnt help her get up and called another nurse for help. I had to pick up my grandma from the floor on to the wheel chair. 3 nurses came to help but it took them so long to come . Also when i was there visisting they never came when you pressed the button. I talked to the Nurse Director when my grandmother fell and they didnt even write a report she just said she was sorry. My grandma didnt get breakfast one of the day she was there and she was forgotten no one came get her up from her bed ulntil later on and she also was yelled at and discrimated because of her infection. Please dont leave your family there they dont care.

I was under the impression that my mother was under the care of green valley health but she wasn’t even there and NO ONE called to let me know where she is. That is not acceptable and is against protocol. Management needs to reach out to me regarding this issue.

I was a patient here and while it's I absolutely beautiful and has a lot of things to do the quality of care was the absolute worst ever. I was put in a room and literally forgotten about. I would press my call light and it would get shut off at the nurses station this went on for days. When I was finally able to tell staff they blew it off. I would not recommend this place under any circumstances.
