Very disappointing experience. They refused to perform a post-accident drug test, stating they only do collections until 2 PM—information that was not updated in FormFox or i3Screen. We even offered to pay for an after-hours collection, but they still declined. I would strongly recommend avoiding this location.

Well first off Awesome place being im from Cali. I had a bit of concern but Dr.David told me to come on in and let him review my paperwork. Arrived checked in and left in under 30 mins. The doc reviewed my stuff and said you’re good. Finished the exam and done.

The best part it was $89(cash) for a D.O.T physical.

Will repeat again if in area and need one

After having a bad experience at Concentra my Operations manager referred me to this place. And honestly I will never go anywhere else. Great people and Doctor! The ease of getting in and out was fabulous. I highly suggest getting a fabulous! We will definitely use the office again.
**Request a hard copy of your medical certificate.
Though the office electronically submitts your forms, there are 5 states that are not set up yet and your license will be downgraded and you can be OOS up to 10 days. My husband's personal experience.

Been testing with this place for two years! Called them from Texas, and asked could they email me a copy of my results they replied NO! The weather is bad! And didn't even try to email me. Just went in on 12/21/23. Everyone was very helpful and ready to go. I was worried it would take me long as a walked in, but that wasn't the case as soon as I walked in, they helped me and did it very efficiently. The staff is amazing and the doctor is amazing. The people that work there in the front office was wonderful. Both of the ladies was awesome. Was there where you needed? It was very quick also with the doctor was very very thorough. This place is on Craig & I-15, 20 min. away from my home... worth the drive! I even got a Veterans discount off the price.Paid $79 ($10 less than adv. $89). I'm a 15+ year CDL holder.This is by far the best, fastest(20 min. in-out), clean, easiest & most helpful examination clinic I've ever been to. I made the appt. the day before without a required verification email or call ! WoW, the place I 'tried to use' before this one was ProCare Med.@ Nellis & Sahara, near me was "HELL". Wouldn't answer the phone, left messg, No call back, when they did, too late. I went to that place in person. They had NO RECEPTIONIST... the 8 people in the waiting room looked like death warmed over! lol. Asked em how long to wait. They ALL said "forget it".
"DOT Express" was the opposite; New, bright, truck parking. I did paperwork before my appt. time, was escorted in as soon as this "short form" was done. Took my basics, sat me down, Doc walked in & even helped me to retest without the new glasses I had gotten to correct my eyesight, I passed without the glasses!!! Meaning I didn't have to go to DMV & put "glasses" on license. ugh!
Also I asked to "rest for 5 min." before taking Blood Press., they said "OK" without a hitch... makes a BIG DIFF. for people like me, who have the "white coat syndrome". My BP dropped significantly after the 5 min. wait!!!
Yall, I don't tell all the good stuff unless I'm trying to help. This place is the RIGHT PLACE FOR SCAREDY CATS LIKE ME...They don't pay me for this review... JUST HIGHLY RECOMMENDED FOR A FAST EASY, CHEAP, CDL/MED. exam. Thank-you to Dr. & very professional staff, I will only come back here in 2 years... ;-)
