I went in with extreme pain and was told I would have prescriptions at my pharmacy and that was 4 days ago and still nothing. Ended up having to go to the emergency room after seeing my primary doctor for the first time. Didn't give me any information on what might be wrong which I understand considering my blood work and imaging wasn't done but still as a doctor I would never tell my patient I'm going to prescribe them something and then not do it the day beforr a holiday 3 day weekend especially if the patient was in agonizing pain

Went in specifically for asthma care and requested a fill on my medication, doctor didn’t even look me in the eyes, definitely a lack of patient bed side manner in his training. I specifically asked if ALL my medications would be put in to pick up at my pharmacy since my asthma has been acting up pretty badly and was “reassured” they’d all be ordered. When I arrived at the pharmacy I only had 1 order for one medication. I was expecting 3. The doctor hadn’t put any others in? The best part of the office was the nurses and medical assistant who were kind, approachable and seemingly helpful, it was the first thing I noticed. The Dr….. strange. I only review because I’m tired of less than sub par (and insanely over priced) medical care In a “first world” country, where most drs seem to have no control, idea, or care. People of all kinds should be cared for with rigor, especially if this is the field you spent so much money and time to be in, do better. It’s healthcare, aka caring for the health of other human beings. Why are you over charging and under caring? People deserve better. (I have insurance thank goodness, but ik its not as easy for other in regards to payment) advocate for yourself, bc most of these drs won’t. Don’t except care that’s less than what you deserve, be self aware and informed before stepping into any healthcare facility so that you aren’t pushed aside, neglected or taken advantage of by people you’re told to trust with your health and (in some cases) your life. #overinstitutionslizedmoneyfocusedbusinessesschemes

I’d give Hale Family Practice a solid 3 out of 5 stars. The staff and doctor are friendly, professional, and clearly knowledgeable, which makes visits feel comfortable and reassuring. However, wait times can be frustrating. There have been multiple occasions where I’ve had to wait 30 minutes or more past my scheduled appointment time. It seems like this may be due to only having one doctor available at certain times of the day.

Overall, the quality of care is good, but improving scheduling or reducing wait times would make a big difference in the overall experience.

I’m a new patient at Hale Family Practice and the people here are so very nice!! The physician is very humble and very nice mark joel culanag I can’t recommend enough.The phlebotomist was amazing as well!!! It’s very hard to get blood from me to the point where other places have to take it from my foot. I even asked Mark because he was passing if she could just take it out of my foot . He was very compassionate and he said we don’t want to do that cause it’ll create blood clots. It didn’t even matter the phlebotomist got it on the second try and I haven’t had anyone get it on the second try ever . no I would highly recommend this place The front desk girls are also extremely nice and pay attention to detail.Amazing people here at this office and I look forward to all my up coming visits!!! I have visited this office three times, and unfortunately I am not satisfied with the front office staff. During my last visit, I noticed the front desk employee’s nails were very long and looked unclean, and dark nail polish which I feel is not appropriate for a medical office environment.

Additionally, the nurse practitioner Mr. Mark, always wearing a mask the entire time and it was very difficult to hear or understand him. Clear communication is important, and this made the visit frustrating for me.

Went there twice over a year ago and now they won't stop sending robo calls. You can unsubscribe, tell them to stop, and block them, but they'll keep trying. Very annoying so avoid them if you don't want more spam. They also nickle and dime you for everything. Instead of just emailing you the results of your lab, they charge you a full appointment for a 5 minute chat.

This is my second visit to Hale Family Practice. I have now visited both locations. Both locations are extremely rundown, the walls are bare, the rooms are tiny and there’s barely enough space for two people to move around comfortably. The only reason I returned was because I needed a medication refill before the typical one year exam requirement, and I couldn’t get into a new office for several weeks.
During my first visit, the doctor didn’t perform any kind of physical exam you normally expect. It was just a quick sample collection and I was sent on my way. At my most recent appointment, the doctor did examine me but not gynecologically And you spoke so quietly that I could barely understand him. I found myself having to guide the conversation, ask the right questions, and essentially advocate for my own care.

This office and almost all of its workers are extremely unprofessional, patients are constantly having to remind Mr.Culanag and the people working up front on information, appointments or paperwork, it’s like they’re not even going back and reading or updating our notes unless we’re repeatedly calling them or have to pop up at the office to remind them of their jobs because they barely call us or they simply just don’t answer their phones. Nothing is ever ready in a timely manner and they offer little to no customer service. They cant seem to get my referral approved by mt insurance becasue no one has access apparently. Been trying to go see a specialist and my insurance is asking for more info but isn't getting it. I went there 2 years ago. I paid all of my co pays. When they charged me it was for my copays - which was paid. Then I took all the paperwork to the manager Jackie, she was going to give it to Mark to fix and she didn’t. Every bill I would call or email about. I even tried leaving reviews to get their attention. This was absolutely never taken care of and then they sent me to collections. I had a televisit scheduled for today. I was sent several confirmations via email and text. Appointment was also made via phone with a representative. Not once did I anyone mention I had a balance on my account. I never received anything notifying me that a balance was due whatsoever. 15 minutes before the scheduled and confirmed appointment, I got a call stating there was a balance due and that the dr will not get on the televisit until paid so we can reschedule. This is extremely unprofessional and an inconvenience. The problem is not that there is a bill due. The problem is that they don’t notify you to even make you aware. To say the dr will not come on the call has me feeling a way because it’s not even like I knew about the balance. Very disappointing that they didn’t even care.

What a horrible place! They never answer the phone The doctor gave me a referral and they never fax it to the specialist I have been calling for one month and they have not send it. You call and and leave a message and the never return your call they simply don’t care. I went for a knee pain he sents me to get a MRI and it showed I have a tear I was in a terrible pain I called the office and of course they didn’t answer the call I left a message to let them know I was in a lot of pain and if they could call pain medicine to the pharmacy and they never returned my call. I confirm all details, send all my medical specifics, and insurance information within an hour of booking the appointment.

I hate writing a bad review, but I've been bounced around too much. I initially went to their Henderson branch to establish a PCP after moving here. I was then told to come to this location and see a PCP who was authorized to send in my prescription. They sent my initial prescription out, but the pharmacy sent it back. I've been spending a month trying to get them to send it to a second pharmacy. They've just told me I must now make another appointment to get it sent out again... I'm going to try my luck somewhere else.

I believe this Dr is only interested in prescribing Ozempic and other Weight loss medication’s for profit. I waited three weeks to get a refill on my Chantix.. CARE QUALITY:
The NP does not want to advocate for patients to get medications that are needed and is not good at following up with patients.

My partner has been going to this clinic for about 1-2 months and they gave him the run around. I had to help call to reschedule appointments because they would always call him while he was at work.

We gave them an extensive family history, did all the blood work, tried holistic remedies to assist with weight loss, made lifestyle changes (exercise, walk more, better sleep schedule) and explained any side effects such as chest pains, fatigue, or any other adverse effects due a pre-diabetic state.

Medications denied, no follow up, prior authorization denied, and when asked why, the NP will only say “I don’t know” and did not provide any follow up care for diabetes prevention.

The NP said to circle back once my partner’s A1C is in diabetic range! He has insurance and we want it to be approved BEFORE he becomes diabetic.

And on top of all that the NP kept pressing a cash pay for Mounjaro and Zepbound of $499 and to put him on Phentermine. My partner is prediabetic and has an extensive family history indicating other risk factors that are GENETIC. Highly Disappointed

Came into today for a doctor’s appointment and was once again blindsided by talks of owing them money for over $200. Explained to the receptionist that I was confused as to why this was not brought up when I made the doctor’s appointment nor was it brought up when I was in the doctor’s office 3 times in December of 2024. The receptionist did her best to explain it to me as I was having her update my insurance but being that I used to be in the medical field myself I just asked for the documentation so that I could look for myself to see what was the issue.

One visit wasn’t paid for because the doctor used the wrong diagnosis codes and plain as day it said the codes don’t cover me for what he was testing me for. So I ask for more documentation to see why. Looking at the notes from my last visit was also a little disheartening because I saw that they wrote down certain points but didn’t listen nor note my primary concerns so of course it looks like I’m getting tested for no reason!! Then they also noted that I paid for one visit but even though it was seen it was apparently charged to the wrong day (or not applied at all.)

So then I basically get told if I don’t make a payment (for questionable charges???) I could not be seen today because the doctor insists I pay something before being seen but I can’t even get an accurate reason as to why some of these charges exist. So now I have to go through my insurance to get more information as to why these weren’t covered and I know it’s due to the mistakes of the office. To add insult to injury, I was told if they figure it out they will “try” to get me back into the office to be seen. I don’t live the closest to the office and after all the hassles I’m not even sure I want to go back. It’s just sad because I made this appointment due to being in full body pain for weeks to have to deal with confusion and what seems to be a bit of negligence.

It’s so hard to find good doctors now but these kind of issues just makes it harder. Very disappointing.

NP Lopez-Culanag is great, attentive and thorough, but the front desk is completely unresponsive. I’ve called multiple times with urgent concerns about labs and medication refills and never got a call back. It’s unacceptable, and I won’t be returning. Avoid At All Costs!

I went to this practice on 3/30/25 for a routine paper smear. It took me almost 2weeks to get the results, because they never answer their phones or portal messages.

I had to call from my WORK cellular to even get them to pick up the phone. Then, they tried to charge me a secondary co-pay to go over my RESULTS that I read for myself. Even when the Nurse Practitioner finally came on, she never read the messages or the results that I read for myself. The worst part of all???

She goes on to prescribed a TOPICAL gel instead of a VAGINAL gel. If I had not read the difference and actually trusted her, I could have HARMED myself.

Reading the few bad reviews there are of this office is quit shocking. I have never had issues of getting ahold of them via phone etc. They have always listened to me and my concerns. I’ve been going here for over a year now and even when I moved, I still make the drive just because this is the best office I’ve gone too.

you schedule an appointment and wait over two hours to be seen even though there’s nobody else in the waiting room
And the doctor there Mark Joel Culanag stole money from me, he told me to cash app him then said he never received it even though I showed him proof the money was clearly taken from my account! I recently made an appointment for health concerns and set up an appointment with office. Office was friendly and clean and made me feel not judged for the reasons I came in and speaking with NP was smooth and easy got to the point and handled everything. I’m giving 3 stars simply for the staff. The girls are very kind, helpful and accommodating when it’s necessary. I have been coming here for over a year - might be closer to 2 years now. The first visit was wonderful. However, over time, I realized that every time I have an appt Mark has no idea who I am when he walks in the room. He asks me every single time if I’ve ever been there before. If he read my chart prior to entering the room he would be much more informed but he doesn’t which raises the question of him being too rushed to effectively treat his patients.
The last visit, this past Friday, will be my last one. Not because of mark or his staff (though I’d appreciate it so much more if he remembered who I was instead of having me repeat everything from prior visits). It’s because of the girl that drew my blood - or at else tried to. Ashely came in after waiting about 20 minutes in the room and said she was going to do the blood draw. I knew it wasn’t going to go well when she didn’t even know how to tie the band around my arm. I showed her the vein that is always used without a problem in my right arm. She claimed I was too dehydrated to get blood from. I drink nothing but water so that’s not true. She then switched to my left arm - that no one ever uses - and the same thing happened. When the blood stopped coming out she literally fished around in my vein for a good 5 minutes or so until I finally was bleeding all over. Literally - blood all down my arm and on my hands. It was horrible. Welp Ashley - there’s the blood you claim you can’t get. This is HIGHLY concerning to me. If you’re having someone draw blood who clearly has no idea what they’re doing then are they even taking property safety measures with the needles. Drawing blood is nor a joke - which she seemed to make it. Now I have two very large bruises on BOTH arms. So that looks great when I have to go to work on Monday. After this ordeal she hands me the orders and tells me to go to either quest or labcorp as soon as possible. SO - like them, I too have a job I must show up for. My job is very busy and it isn’t easy getting off for a doctors appt. I simply do not have the time to go do this all over again when I did what I was supposed to do by showing up to my previously scheduled appt. Now they don’t have enough blood to test it so there’s no way they can know if I’m even on the right dose of thyroid meds. Thyroid issues can become serious. I don’t joke about my health and I don’t appreciate a doctors office doing so either. Again - mostly just Ashley. I won’t be returning because of this. I need a doctor that will listen and remember me and not rush me. And I need a Dr that has competent staff that won’t waste my time like this.

My experience with Dr. Mark was disappointing. He appears to be quite forgetful, often failing to recall tests he has ordered for his patients. Additionally, he does not seem to review prior notes before appointments, requiring patients to repeatedly remind him of past visits and discussions. His approach lacks attentiveness and genuine concern for his patients’ well-being, which raises serious concerns about the quality of care provided. Based on my experience, I would not recommend Dr. Mark.

If I could give less stars I would. Mark is an extremely unprofessional and insensitive physician. I waited over an hour for a video call appointment that lasted 3 minutes for an adhd diagnoses that got me nowhere. He asked me if I tried de-stressing and taking things off my plate as well as saying I don’t have adhd because I’m not failing in any of my classes. He asked me a total of 5 personal questions before telling me it sounds like I’m stressed and need to see a psychiatrist for an ADHD evaluation and that it sounds like I’m not struggling at all. A complete waste of time and an unbearably disappointing visit, will NEVER be seen by this physician again and will further recommend people to never go to this doctor

Doctors great but the office staff is terrible . I was patient of his 3 years ago and they updated my information at the front desk . Waited a whole hour for my appointment and for them to send the medicine to wrong and to the wrong laboratory to get my blood work . Still 3 days after my appointment I have no medicine and I have called more an 5 times to figure out what is going on . The office staff wants me to wait a week for my medicine and not inform the doctor in person but by email . They made a mistake and I am paying for it . It always sucks when you have a great doctor and terrible staff . Switching immediately do not go here if your sick or take medicine

It was a good first visit. For me, anytime i meet someone for the first time, there is a "feeling" out process. I believe with each visit i will feel more comfortable and i will be able to talk more freely. The awkward first visit was more on me than NP Culanag. At hale Family practice the staff was awesome they were on top of everything every need Dr nurse practitioner Mark is the best so far I was looking for a good clean family practice and I'm glad so glad I found Hale Family Practice.Again dr.mark took the time for a televisit with in a hr.after I called him to let him know I was out of my heart med.and couldn't see my cardiologist for a whole month and I'm not supposed to go with that medication I could have blood clots and lead to another heart attack so he immediately went ahead and prescribed for another month until I could see my cardiologist from my medicine he's an awesome doctor he understands and he's got great compassion for his patience. Today I just had a televisit to go over my pap.and blood work.im ok on both .when the pap was been done I was a lot nervous but Dr.Mark and his assistant was gentle and understanding because I hadn't had a pap in 25 years.im 62yrs old .and the lab tech nurse was the best she listened to me I don't have veins that show and hard to get because the are small and deep but she only poked me once and when she was done I didn't even feel the needle left no bruises .she was great.once again had the best health care .thank u Dr.Mark and staff

I couldn’t be happier with my experience! Finding a PCP who truly listens, shows genuine care, and takes the time to explain what’s covered and what’s not by insurance is incredibly rare these days. Many providers seem uninterested or rushed, but this visit was completely different. The staff was also incredibly friendly, which made the experience even better. Another great thing is that they can do blood work right at the office, so there’s no need to hustle to a separate lab for blood or urine tests. It’s so convenient! Thank you for taking the time to listen and making my experience exceptional!

I’ve been a patient at this office for two years, and I can honestly say I’m extremely disappointed with the service. It’s nearly impossible to get anyone on the phone – I’ve spent countless hours trying to reach someone, only to be left on hold or never get a call back.

I’ve been requesting my health records for over two months now, and despite multiple calls, follow-ups, and promises that “someone will get back to me,” I still haven’t received them. It’s frustrating and unprofessional.

At this point, I’ve lost trust in their ability to manage basic tasks, so I’ve decided to switch providers. It’s unfortunate because I was hoping for a better experience, but I can’t keep wasting my time. Very disappointed with the lack of care and responsiveness.

I spent an entire day trying to get them on the phone and get this sorted out… I will try again tomorrow.

Was waiting over 2 hours for my visit because they were requiring I make Mark Joel my PCP despite it being my first visit before I could be seen. I had to call insurance missed my visit and wait till he was done with another patient or whatever he was doing when it was all said and done this visit all took about 3hours.... The young girls in the office are disorganized and not synchronized enough to work there. Smh.

Next, I was seeing Mark Joel for some simple med refills because my last PCP whom I had been seeing for 15years recently retired. Mark Joel questioned everything... Why I needed those meds.. Um. Doesn't he see in the system what I've been taking and from the paperwork I filled out what the appointment was for?? Two of the meds are literally non controlled substances (heartburn and for muscle spasms) and he made me get up and show him my back as to where it was hurting and he wanted to know why after I explained I had complications with a c-section .. I guess he didnt like my answer and immediately ordered xrays (which I'm not going to do) I didn't like his approach at all. He kept questioning me about why I needed heartburn pills as well... I explained it's because of certain food I eat that I try to avoid in which he gave me the lowest dose possible (20mg) Geez, thanks! He made me feel uncomfortable and I felt he was rude and condescending. The last medication I asked for which was for insomnia (ambian) he wouldn't give me unless I came into the office and took a drug test despite me telling him I had just taken one for my other dr less than 3 weeks ago. Plus he said after all that he would only be doing the ambian for a very short time. Lol, Ummm, what?? I've been on my sleep meds for 10years and I know what works for me and my body. He seems like a know it all and didnt really listen to what was saying. Even though he has no psychiatrist training he said he was gonna talk to my psychiatrist and see if we can raise my antidepressant to help with anxiety for sleep so I wouldnt need sleep meds. Umm, no your not. My body my choice and I will not be seeing him again. I am calling today to remove him from my insurance as my PCP. He seems very anti meds so I would avoid him and this entire office if you need any sort of sleep med or other controlled substance on a regular basis. From the reviews it seems he does mainly like "quick care" like for ear infections ect. Mark Joel needs to learn to listen more and stop trying to push certain meds he's compensated for by pharmaceutical companies for prescribing and pushing his opinions on his patients. It's going to be a giant no from me. I would never see him again.

Also be prepared to be bombarded with spam because his office will send out 20 automated messages to phone & email about your upcoming visit. I guess we can't be late but he can.

I have never had trouble getting controlled substances through telehealth with my proven history I have been previously prescribed them for years. Again I marked the reason for visit as refill for (ambian) why see me if you couldnt help? Because I have Medicaid and you know you could charge it up for an unnecessary visit? Number 2, Why would I make someone I've never seen or know if I even want to keep my new PCP? Never had to do that before.. If I was satisfied with his services, that could have been done in the future not 1st visit. Also, I see my psychiatrist weekly. He is very well aware of my anxiety, my needs and the meds I take. He is not requesting I up my medication so why would Mark Joel make that suggestion? Hes not a psychiatrist. I simply needed a refill I have been taking for 10+yrs. If that is not possible with Hale Family Practice I will find a new PCP so now that I will not be seeing him again it makes no sense I have to re call in to my insurance and say never mind he isnt my PCP anymore please remove Mark Joel. Big waste of time you see? Also, I informed Mark my psychiatrist actually recommended I ask my PCP for the (ambian) I'll go elsewhere.

In my exhaustive search for a psychiatric provider who is taking new patients, accepts my insurance (ChampVA and Tricare), who offered both in person, as well as telemed visits and most importantly has/had appointments that were not 3-6 months out but rather same or next day. I was lucky enough to find Hale Family Practice and Dr. Mark on ZocDoc. Dr. Mark is by far one of the best providers I have seen since moving to Nevada a little over 6 years ago. I would even go so far as to say one of the best psychiatric nurse practitioners I have ever had the pleasure of working with. I was able to make a next day telemed appointment in which he reviewed all of my records both physical and psychiatric. He took the time to go over why I was switching from one of my previous providers for my psychiatric medication management, what has changed in my life over the last several days, weeks, months, and even the last two years. What changes I was looking to make, what I have tried before and did they work or were there were any side effects, what he is able to provide under the law, contraindications with current physical medications I am taking, my physical health and what other services he might be able to assist me with. He even provided me with several referrals. All new patient paperwork was able to be completed ahead of time online via a link that was texted to my cellphone. He was incredibly friendly, knowledgeable, understanding and makes you feel like you are really being listened too. The only downside was that because he was prescribing a controlled substance for ADHD, I did need to go into the office to wet sign a controlled substance agreement and to take a urine test. Not uncommon. However, I do wish I had been told this ahead of time as I would have just scheduled my first appointment in person. To remedy this though, Dr. Mark did not have me make another appointment but rather, offered for me to come in right same day (it was 3:30 at the time and the office closed at 5) or to come in any time the following day that was convenient for me. When I went in the next day the staff was very kind, they knew I was in a rush so they got me in and out. Dr. Mark also made sure to take some time out of his schedule to meet and speak with me in person. My prescriptions were already at the pharmacy being filled before I left his office. As a medical professional myself I highly recommend Dr. Mark and Hale Family Practice for all of your medical needs!

I came here with a concern and they suggested I get an ultrasound. I got one on a Friday. One week later, I hadn't heard back with results. I called them. They had had my results for 5 days and had not contacted me. They EMAILED me my results with no explanation. Trying to get an explanation was like pulling teeth, they kept passing me around and eventually someone said they would call me. I didn't get a call back until someone called me telling me that I didn't call them?!?!?!? I was told someone would call me. Horrible horrible patient experience. They do not care and I do not recommend. Literally when I called asking for my results someone should have been able to take 5 minutes to explain my results. It is NOT THAT HARD!

They charged me after I left the practice. I sent in a the paperwork showing I paid and they were going to correct it. I still get the bill it was never cleared. While I think the staff is nice, it doesn’t matter how nice they are if they are billing after you already paid. Would NOT RECOMMEND
